The Best Electrolytes for POTS: What to Look For on the Label

Read the label like a clinician: sodium per serving is the number that matters, then weigh sugar, potassium, and third-party testing.

Most people with POTS are told some version of the same thing: load up on fluids and sodium, every day. Then they walk into the electrolyte aisle and find dozens of brightly packaged options that all promise “hydration,” with very little to help them choose. The marketing is loud; the labels are the only thing that will tell you whether a product fits a POTS protocol. This is a guide to reading them the way a clinician would.

Sodium per serving is the number that matters

For POTS, sodium is the headline metric. Protocols commonly cite a daily target in the range of 6 to 10 grams of sodium, which is roughly 15 to 25 grams of table salt. That is several times the general-population sodium recommendation, and it is the reason a clinician points patients toward electrolyte mixes in the first place.

Once you know that target, the label tells you almost everything. Flip the package over, find “Sodium” on the nutrition panel, and read the milligrams per serving. Most consumer “hydration” mixes deliver somewhere between 200 and 500 mg of sodium per stick. A smaller number of specialty mixes, often marketed toward endurance athletes or people with autonomic conditions, deliver 1,000 mg or more per stick.

The math is unforgiving. If your clinician has you aiming at, say, 8 grams of sodium per day, a 300 mg stick is under 4% of your target — you would need more than twenty. A 1,000 mg stick gets you 12.5% in one go. Neither product is “better” in the abstract, but for orthostatic loading they are not interchangeable. Always do the per-stick math before you compare prices.

Sugar load — and why most sports drinks underdeliver sodium and overdo sugar

A lot of well-known sports drinks were engineered for a different problem: replacing the glycogen that endurance athletes burn through during long efforts. Sugar does real work in that context, and the sodium content was set to roughly match what a runner loses in sweat — not what a POTS patient needs to defend blood volume against gravity.

That mismatch is why a typical sports drink disappoints on both ends of the label. The sodium is modest (often 150 to 300 mg per serving) and the sugar is generous (frequently 20 to 35 grams). You end up drinking a lot of sugar for a fraction of the sodium you came for.

POTS patients tend to do better with high-sodium, low-sugar formulations. A small amount of sugar can help with palatability and, in some formulations, with sodium absorption — but the sugar is not the point. The sodium is the point. When you scan a label, sugar content above what you would tolerate in a soft drink is a sign the product is solving someone else’s problem.

Potassium ratio — useful but secondary

Potassium gets a lot of label real estate because it sounds important and reads as “balanced.” For most people with POTS, it is secondary. Volume defense is a sodium problem; potassium depletion is uncommon in this group unless something specific is going on — heavy endurance exercise, certain blood pressure medications, or a clinician-flagged lab.

A modest amount of potassium in a mix is fine and probably helpful, but it is not a reason to choose one product over another. A mix that brags about its potassium ratio while delivering 250 mg of sodium per stick has its priorities backwards for this use case. If you are on a medication that affects potassium, that is a conversation with your prescriber.

Third-party testing and what to actually verify

Supplement labels are not policed the way drug labels are, so independent verification is worth looking for. Three marks come up most often:

  • NSF Certified for Sport — tests for banned substances and confirms label accuracy.
  • Informed Sport — similar scope, batch-level testing for banned substances.
  • USP Verified — confirms identity, potency, and purity against United States Pharmacopeia standards.

What these marks do promise: that what is on the label is in the packet, in roughly the amounts claimed, without contamination from substances the testing program covers. What they do not promise: that the product will help POTS, or any other condition. A clean, accurately labeled mix with 200 mg of sodium per stick is still a 200 mg-of-sodium product. Use the testing marks to trust the numbers — then judge the numbers on their own.

Other things worth a glance

A few more lines deserve a quick look:

  • Magnesium. Often marketed for sleep and cramps. Evidence for either claim in the general population is modest, and POTS-specific data is thinner. A small amount is unlikely to hurt, but it should not drive your choice.
  • Additives. Artificial sweeteners and citric acid are common and well tolerated by most people, but some POTS patients have sensitive stomachs and react to one or the other. If a mix consistently bothers you, scan that part of the label.
  • Serving size sleight of hand. Some products list nutrition “per serving” while quietly defining a serving as two sticks. Check the serving size against the stick count, and recompute sodium per stick if needed.

Sodium loading is a real intervention, and doses should be discussed with your clinician, especially if you have hypertension, kidney disease, or heart failure. The right daily target — and how to spread it across the day — varies from person to person.

The “best” electrolyte mix for POTS is not a brand, it is a label that lines up with what you actually need. Find sodium per serving in milligrams and compare it to the daily target your clinician has set. Treat sugar as a cost, not a feature. Take potassium and magnesium as bonuses, not deciding factors. Use third-party testing marks to trust the numbers, and read serving sizes carefully. Once you can do that in the aisle, the marketing stops mattering and the right product becomes obvious.
